Applicants must obtain

• Statewide Background Screening, Level 2
• Professional Certificate of Completion (CNA, LPN, RN) with curriculum  
• Certificate for Home Health Aide/ 75 hours for Home Health Aide.

• I.D. / Drivers License                
• Proof of Residence / Employment Authorization
• Social Security Card

• Auto Insurance (if applicable)

• Physical (including TB or Chest X-ray) within the  past 6 months detailing that you are “free of communicable diseases.”

• CPR Card
• Initial (Original Certificates) and C.E.U.’s (New Certificates) for HIV / OSHA and Domestic Violence
