• Statewide background screening
• Professional Certificate of Completion (HHA,CNA, LPN, RN) with curriculum
• I.D. and Social Security Card
• Proof of Residence / Employment Authorization
                   or
• Proof of Citizenship: Passport, Birth Certificate, or Naturalization
• Auto Insurance (if driving)
• Physical (including TB or Chest X-ray) within the  past 6 months detailing that you are “free of communicable diseases.”
• 3 Business References
• CPR Card
• Initial (Original Certificates) and C.E.U.’s (New Certificates) for HIV / OSHA and Domestic Violence
